Advanced registered nursing graduates are entering the profession at dynamic time when roles and scope of practice are shifting based on developments in legislation and policy in response to the evolving needs of the health care system. Professional nursing organizations play an important role in making sure the perspectives of advanced registered nurses are heard, and in supporting nurse specialties in their efforts to expand their scope of practice and their full participation throughout the health care system.

For this assignment, you will conduct research on the current scope of practice for your specialty and efforts that are being made to expand that scope and the role of the advanced nurse in positively influencing the health care system. Write a 1,250-1,500-word paper that includes the following:

  1. A discussion of the scope of your future role as an advanced registered nurse- NURSE EDUCATOR, including any regulatory, certification, or accreditation agencies that define that scope.
  2. A discussion of three professional nursing organizations that you think are most influential in advancing the scope and influence of advanced nursing. Of these organizations, evaluate the one that you would most like to join. How do its goals and mission fit in with your worldview and philosophy of care? How might membership in this organization improve your practice?

Introduction

Advanced registered nursing graduates are going into a nursing field that is changing fast. The following analysis discusses the scope of an advanced registered nurse-nurse educator, the regulators, and certification as well as accreditation bodies in place. It also discusses three key professional nursing organization and why American Nursing Association ANA stands out. Finally, it explores the evolving issue of nurse shortage and how it is going to affect the profession. With the increased responsibilities, an advanced nurse needs to have a good understanding of the scope of their role, the organizations to join and evolving issues and how they will affect their practice.

Scope of Advanced Registered Nurse – Nurse Educator

As a nurse educator, one is expected to use their experience and knowledge to bring knowledge as well enthusiasm in the process of training and preparing the students to be great nurses for generations to come (Hall & Mast, 2015). The role of nurse educator includes facilitating learning as well as using assessment and evaluation strategies to gauge how ready the students are for a future career in nursing and guide them accordingly on their areas of weakness. Nurse educators also participate in curriculum design and also help in evaluation of program outcomes. It offers two nurse educator certifications which are certified nurse educator and certified academic nurse educator. For the certified nurse educator, they facilitate learning by teaching, evaluating, and curriculum design. Certified academic nurse educators facilitating learning for students using clinical components of the learning program.

Nursing Organizations

There exist many professional nurses’ organizations for one to choose. One of those organizations is the American Nurses Association and it is one of the oldest and largest organizations in the in the country. Members receive many benefits from joining such as accessing an online library with various academic sources that relate to nursing (Mathews, 2015). It can be very helpful especially for those nurses that are involved in study or research activities. Members also get access to webinars on various topics that cover various areas of their profession. Members also get to save money on areas of certification and publications as they receive special offers. ANA is also credited with the development of the code of ethics for nurses with interpretive statement which is a huge source of guidelines on the steps that nurses need to make in ensuring that they operate ethically and helps to promote the scope of advanced nursing.

A second organization to consider is the hospice and palliative nurses association. The organization focuses mainly on hospice and palliative care as its name suggests. Nurses work in areas where dealing with death can be an often occurrence and they find being a member of this organization helpful as members support each other (Catallo, Spalding, & Haghiri-Vijeh, 2014). It provides a strong support system for nurses especially during the tough times when they lose patients or are exposed to a lot of human suffering. The organization allows members to have access to various professional journals that touch on areas of hospice and palliative care. Most of the members that join become either mentees or are mentors to others. Other benefits that members get include training on leadership skills and learn on approaches to better deal with patients and colleagues at work.

As a nurse, it is natural for one to be attracted to ethics and human rights. The organization has it as one of its core areas and even has a center of ethics and human rights. Being a member of the organization therefore allows one to be a part of a group of people that are doing something to make the world better in matters of health but also ensure that human rights are protected and promoted.

One of the impacts of the issue is that there will be shortages and the existing nurses will be required to work for longer hours and do a lot more work than they would have had to do. This should matter to advanced registered nurse because a lot of them will be having supervisory responsibilities. They will be required to come up with approaches that will help offset the problem and ensure that Americans continue to receive the nursing care that they deserve. Other stakeholder will also need to be involved such as government considering allowing foreign trained nurses to have easier chance of working in the USA

Conclusion

Advanced registered nurses-nurse educator has the responsibility of providing the USA with nurses with the right skills and abilities to perform their duties. However, the existing regulatory, certification and accreditation agencies are doing a great job of assisting with this goal. In America there exists numerous professional nursing organizations to choose from. However, ANA easily stands out from the rest mainly because if its stand on issues of ethics and human rights. In the field of nursing, it is expected that there will be huge shortage in nurses before 2030. Professionals and stakeholders need to come up with approaches to fix the situation while at the same time making do with the limited numbers of professionals’ nurses at their disposal. The future of nursing has its challenges but advanced registered nurses will hopefully be a part of the solution to these problems.

Enacting the bill would result in better healthcare services since it will be addressing some of the challenges related to healthcare that the American people face. AHCA would be better relative to AHA for some reasons, which include:

  • Increased flexibility since not having health insurance would not attract a federal penalty, thus making AHCA more flexible.
  • Tax credits to aid in the payment of tax premiums, which tends to be particularly important for low-income earners. Furthermore, these credits phase out as incomes rise, which in turn increases equality.
  • The introduction of a maximum subsidy or tax credit the government can pay. Such ensures that public resources are utilized in the best way possible.
  • People can buy plans that do not cover benefits that they may not want. Such increases the flexibility, which in turn may lead to increased insurance coverage in the country (Fielder et al., 2017).

For the above reasons, therefore, it is my believe that enacting AHCA and repealing ACA would be more beneficial to the American people considering the former addresses new healthcare challenges, increases flexibility, and results in a better use of public resources relative to the latter.

The rationale for the position that I take in the letter is that AHCA offers a better opportunity to improve healthcare in the United States compared to using the current Obama Care are system. Such would result in better uses of resources for other critical elements, such as making healthcare affordable for all people, and not only those who have insurance cover. Moreover, the cost of healthcare in the country may decrease because of introducing maximum credits, and paying them based on the income of families, which would contribute to equality (Thompson et al., 2018). For instance, when family or individual incomes exceed certain amounts, they do not qualify for the tax credits or subsidies.

The member was in opposition of AHCA in congress. He was strongly opposed to the bill and strongly campaigned against it. However, such may have been caused by his party affiliation, but not an analysis of the bill fully to understand how it would affect healthcare in the United States. As such, it would have been prudent for the representative to support the bill since it would have been more beneficial to the American people compared to AHA.

One of the reasons for the need for supporting is increased cost of health insurance. While AHA promised to save around $2,500 per family each year, it has not. In fact, the Department of Health and Human Services notes that since 2013, insurance premiums have more than doubled. However, the family incomes have only risen by around 19%, which shows that the rise in premiums is not because of an improving economy (Fielder et al., 2017). Therefore, introducing AHCA may control the increasing cost of insurance premiums, while at the same time offering individuals with more flexibility since they can choose whether to be covered or not, or the benefits that they would like to be covered for.

Furthermore, there is an urgent need to repeal AHA because entitlement spending has increased drastically during the last few years. AHA has caused entitlement spending to increase more than it had been expected. For instance, the projected costs of Medicaid and AHA subsidies are expected to be $4.8 trillion, with both being responsible for an estimated 44% of the anticipated increase in entitlement spending in the future (Williamson & Singe, 2017). Therefore, enacting AHCA would ensure that such costs spending are controlled, which will be beneficial to the economy.

Furthermore, AHCA presents a new opportunity for lowering the costs of healthcare in the United States. With the enactment of AHA, it was hoped that healthcare in the country would become more affordable. However, with the enactment, healthcare costs have continued rising. In that regard, I find it to be difficult because it may make one to lose his direction when he acknowledges the opposing views without refuting it with the same rigor (Winthrop, n.d.).

Their strategic plan focuses on three areas:  quality, patient safety and satisfaction, Olympic Medical Center and community relationships, and organizational performance.

The first strategic objective Olympic Medical Center (OMC) has revolves around ensuring that they offer the safest, highest-quality health care with compassion. They will do so by continuing to standardize their care processes and fortify their systems in order to amplify patient safety and quality, and perfect their ability to deliver positive patient experiences (OMC, 2018-2020, p. 5).  OMC strategies include following evidence-based best practices through national quality initiatives, being a leader in adopting national patient safety initiatives, and encouraging a culture of service that constantly acknowledges the expectations and fulfills the needs of our patients, their families and our community in the services they provide (OMC, 2018-2020, p. 8). In order to implement these strategies OMC will continue to foster a culture of quality, patient safety and experience for their patients and staff. They will concentrate on explicit quality goals that will prevent unnecessary readmissions, and improve emergency department wait times. Lastly, OMC will ensure their Quality Management System will effectively sustain compliance with International Organization for Standardization (ISO) 9001: 2015 standards (OMC, 2018-2020, p. 8).

The second strategic objective Olympic Medical Center (OMC) has is to continue to work with their community partners and representatives and maintain recruitment and retention of the best doctors, advanced practice clinicians, clinical staff and employees to continue to achieve excellence in health care (OMC, 2018-2020, p. 5). The selected groups were then assigned to receive the walking intervention randomly. The sample size was 120, which was obtained after a full 90 days follow up intervention. The researchers’ assigned trained case managers to conduct qualitative methods to answer the research questions. They implemented the intervention sessions two times a week for four consecutive weeks to the targeted intervention group. The measures for primary outcome included the stroke knowledge, mean steps per day for more than seven days, and self-efficacy for reducing the risk of stroke. The measures for exploratory and secondary measures include health-seeking, health-related quality of life, and biological markers for health. The researchers collected the outcome data from participants in the wait-list control and intervention participants three months after complete baseline data collection (Chiu, Bergeron,Williams, Bray, Sutherland & Krauss, 2015). The outcomes were then compared between the two groups using randomized trials data analytic methods. The facilitators and the barrios to the successful implementation of worth the walk were also assessed. The Dietary Approaches to prevent and stop hypertension include incorporation of a dietary pattern that is high in vegetables, fruits, and low in fats, especially dairy foods. Research methods

The researchers used a sample size of three health persons to conduct a randomized crossover trial based on three periods. The individuals consumed a standard Dash diet, a control diet, a lower carbohydrate modification of Dah diet, and a high-fat diet for three weeks each which was separated by two weeks’ periods for washouts (Kwon, et al. 2015). The researchers then conducted laboratory measurements that included the concentration of lipoprotein particles determined by the mobility of ions, which was established at the end of each experimental diet (Kwon, et al. 2015). The researchers used both dependent and independent variables. The dependent variable was high blood pressure, and the DASH and HF-DASH diet were the independent variables that were presented after the tests (Kwon, et al. 2015). The variables were shown on a graph with the high blood pressure being on the y-axes and the DASH and HF -DASH diets being on the x-axes. Each of the researchers analyzed the data using the Statistical Packages for Social Sciences and compared their results to eliminate the chances of personal bias and ensure the accuracy of results.

Results of the study

The participants of the study finished the three dietary periods. The HF-DASH and DASH diets profoundly reduced blood pressure compared to the control diet. There was a reduction in the concentration of medium and large very-low-density lipoprotein particles and triglycerides (Kwon, et al. 2015). The HF-DASH diet also increased the peak particle diameter of LDL and decreased the HDL cholesterol, LDL cholesterol, apolipoprotein A-I, large LDL particles Intermediate density lipoprotein and the width of LDL peak compared with the control diet. The researchers did not mention any limitations of the study.

The Institution Review Board approved the study. Also, the confidentiality of the participants in the study was highly maintained.

Conclusion

The DAS diet significantly lowered blood pressure to the same extent as the HF- DASH diet and also reduced the concentration of VLDL and plasma triglyceride without increasing the cholesterol. The findings of the study can help improve nursing practice in managing hypertension. Nurses should create awareness among patients and the community at large about the DASH and HF-DASH diet to help reduce the mortality rates and the costs of treatments.

The article talks about the church of Lukumi Babalu Aye, a church in Florida whose religious practices differ from others such as the pure traditionalists and the Catholics. Introduction

In healthcare, practitioners often find out that their friends, co-workers, or relatives may request informal medical advice. In addition, people close to the practitioners may ask for medical prescriptions without following medical procedures.  Practitioners realize that engagement in informal prescriptions increase their chances of having malpractice liability. In serious cases, they face legal investigations and risk going to jail or losing their job (Arcangelo et al., 2017). This paper tries to determine the ethical and legal implications that are associated with casual prescriptions. It involves a scenario where a female friend calls for medical prescription while the practitioner does not have her medical history. With that autonomy, the prescription is still written.

Ethical and Legal Implications

The scenario creates an ethical challenge to the practitioner. Due to their close association, the practitioner’s professional objectivity may be affected. The fact that the practitioner prescribes medicine for the client without her medical history shows that the doctor’s judgment is based on their relationship. This act would be done in a polite way, with the practitioner giving an explanation of associated ethical concerns. This may be followed by the practitioner assigning the patient to a different qualified nurse to engage in the prescription. Contrarily, the practitioner may choose to engage in the prescription process. However, the process needs the nurse to have a professional relationship with the patient. The procedure should be done in a facility setting, with the practitioner making suitable medical assessments of the patient. It would need official documentation which would enhance a suitable follow-up (Bao et al., 2016).
