Sometimes clients come into the counseling session with an immediate crisis. They need an immediate solution to a problem. Counseling in these situations needs to be focused and goal oriented.

Jamila has been working with her counselor on learning to better manage her 9-year-old son. Last night, her son was hit by a car and is in the hospital. This has created a number of problems:

1.     She has to arrange childcare for her daughter so she can stay with her son in the hospital.

2.     He will need some special care when he gets home because he will be in a wheelchair.

3.     She is fearful that she will lose her job as a teacher’s aide in the local public school.

Write a 500-750-word paper addressing the following:

1.     How would you prioritize Jamila’s concerns in the counseling session? What would you address first?

2.     What steps would you take in this session to make sure that Jamila has the resources she needs right now?

3.     How would you help Jamila develop a plan to manage the current crisis?

Include at least three scholarly references in your paper.

Crisis Counseling

Introduction

A person is viewed to be in a crisis when they experience a situation which is considered too difficult for them to handle. This issue occurs due to personal inadequacies or insufficient resources. The above factors may make the situation to exceed the individual’s tolerance capabilities. Crisis counseling is therefore a technique that may offer immediate help to an individual in a crisis. To achieve this goal, there are various priorities and models that counselors may use during the intervention (Taylor, & Gibson, 2016).  The essay intends to develop suitable priorities, design ways of creating efficient resources, and develop a suitable management plan for a presented case.

Case Overview

The crisis counseling session focus on a client whose name is Jamila. The client is a teacher’s aide and has two children; a boy and a girl. The son is involved in an accident and needs special care from the mother. In addition, the daughter needs to be placed in a childcare facility to provide time for her son’s care. By caring for her son, Jamila risks her losing her job.

Prioritizing Client’s Concerns

In the scenario, every element is crucial to the client, thereby creating a dilemma to her. From the client’s time and finances are essential concerns for her. Therefore, it would be important to prioritize Jamila’s concerns basing on time and finances. Moreover, it would also focus on the impact of the issue to other issues. The counselor may need to address her job concern first. This is because it would affect her ability to pay for her son’s hospital bill and her daughter’s daycare fees. In addition development of a suitable job schedule may help the client to care for her son without problems. The second issue to be addressed will be son’s care. This concern has been ongoing for the client; hence it is more urgent than her daughter’s daycare concern (Taylor, & Gibson, 2016).

Steps for Attainment of Resources

A systematic approach is suitable in enabling the client to attain resources for controlling the problem. The six-step crisis intervention model will be useful in helping Jamila. In the model, the first step involves defining the problem and understanding it from the client’s perspective. The second step is focused at ensuring the client’s safety. In the scenario, this step focuses on the psychological danger that Jamila may face. Thirdly, the counselor will provide support to the client through suitable communication skills (Silva, Siegmund, & Bredemeier, 2015).

The fourth phase involves examination of different resource alternatives. It may involve assessment of the client’s situational support or involving people close to her. Fifthly, the counselor will engage in making detailed plans and outline the persons and other referral resources that can enhance support to the client. Lastly, the counselor needs to obtain commitment from the client to ensure that the crisis counseling session is effective (Silva, Siegmund, & Bredemeier, 2015).

Developing a Management Plan

Developing a management plan needs suitable strategies. However, there are two broad frameworks that the assistance will follow; nondirective and collaborative counseling. If the client is able to initiate and develop their own management plans, nondirective counseling will be used. Contrarily, collaborative counseling will be suitable if the counselor has established a reliable partnership with the client. A suitable strategy for enhancing proper management planning is creation of awareness. This strategy attempts to bring to the clients thoughts and behaviors that may solve her crisis. In addition, provision of support is essential throughout the six-step process as it validates and expands on suggestions for controlling the situation. Lastly, the practitioner may provide guidance and direction on how to use the resources to manage the crisis (Crosby, Cwik, & Riddle, 2015).

Conclusion

Crises are a common occurrence in people’s lives. However, it is often difficult to manage these challenges without the help of other people. The paper tries to show the role of counselors in helping clients solve their crisis issues. From the paper, it can be shown that using a systematic approach is suitable in helping clients manage their crises. In addition, one can depict that the counseling strategies such as support of clients and offering clients are beneficial tin crisis counseling management.

Problem Overview

            Abusing drugs among adolescents is primarily prevalent in the United States. This makes adolescent years a critical window for the development of substance use disorders. Abusing drugs among the youth influence both their social and psychological development primarily cognitive development (Aldridge, Linford, & Bray, 2017). Nurses and physicians need to intervene early before substance abuse leads to cognitive health issues.  According to Johnston, O’Malley, Bachman & Schulenberg, 2013), physicians need to provide both holistic and quality care to their patients. Through the Road to Evidence-based practice, health care practitioners can find the evidence, appraise the evidence, implement the evidence and evaluate the evidence. Through the use of SBIRT as an evidence-based practice, healthcare practitioners can recognize, decrease and prevent harmful and unhealthy ways of substance use and abuse (Aldridge, Linford, & Bray, 2017). This paper develops a proposal for quality improvement through the introduction of screening, Referral to Treatment (SBIRT) screening tools, intervention, and intervention procedures to adolescent population affected by substance abuse in a health care institution.

Why a Quality Improvement Initiative is Required

The quality improvement initiatives are required because the use and abuse of drugs have become common among adolescents, which make it a critical public concern. The Healthy People 2020 developed critical objectives aimed to reduce substance use and abuse among teenagers (Healthy People 2020, 2018). In addition, most government agencies, medical institutions and medical professional associations have developed free counseling programs and screenings for youth such as initiatives that involve SBIRT integration within the medical health care system in collaboration with the White House Office of National Drug Control Policy, Agency for Health care Research and Quality, Health Resources and Services Administration, National Institutes of Health (NIH), and SAMHSA (Johnston, O’Malley, Bachman & Schulenberg, 2013). Besides, the medical professional associations and international and national public health agencies developed recommendations aimed to guide health care professionals to carry out screening and interventions for individuals especially teenagers believed to abuse and use drugs. Therefore, SBIRT is a critical part that can be placed as part of the Affordable Care Act legislation and Patient Protection Act to help deal with the issue of substance abuse among teenagers in the US (Johnston, O’Malley, Bachman & Schulenberg, 2013). Besides, based on the Periodic Screening, Diagnosis, and Treatment statue, each federal government in the US is expected to provide Medicaid screening assessments for both physical and mental development among teenagers.

Results from Previous Research

According to National Institute on Drug Abuse (2014), drugs such as alcohol, tobacco and other illegally prescribed drugs during adolescent are often abused during the adolescent stage. It is estimated that about 70% of students in high school have abused either alcohol or other drugs while 30% might have abused prescribed medications for non-medical reasons (NIDA, 2014). The desire to fit in and faces new experiences, and deal with peer pressure or problems in school are the primary reasons for abusing drugs.

According to Singh et al., (2017) the factors that influence the use of drugs among adolescent youths include peer pressure and the availability of narcotics in the school, community or neighborhood. The family environment is also a critical factor that may influence teenagers to use and abuse drugs. For example, a violent environment, mental illness, physical and emotional abuse, and the use of drugs in the household may influence the child to use and abuse drugs (Singh et al., (2017). Other factors for abuse of drugs includes the beliefs that drugs are cold, mental health conditions such as ADHD, anxiety, and depression, personality traits such as the need for excitement or poor impulse control, and adolescent’s inherited genetic vulnerability.

The U.S. Preventive Services Task Force (USPSTF) recommends providing routine screening for substance abuse among teenagers between 16 years and 17 years. The USPSTF also found that the use of SBIRT among teenagers is a critical area within the nursing practice that needs to be looked into. According to Aldridge, Linford, & Bray, (2017), SBIRT is a public health approach that is integrated and comprehensive to the delivery of treatment and intervention for individuals at risk of developing substance abuse disorder or for individuals who have a substance abuse disorder. The practice of SBIRT as an evidence-based practice involves screening individual’s suspected of substance abuse and encouraging them to change their behavior through motivational interviews and putting them under a treatment plan. Thus, SBIRT is a simple medical practice that is both effective and brief. Evidence from researches shows that cost-effectiveness and efficacy of SBIRT among adults and its help in managing drug use although is useful, it has mixed reaction. A study by Aldridge, Linford, & Bray, (2017), further states that SBIRT is the evidence-based practice that can help the use of drugs among the youths and prevent cannabis use among teenagers.

The use of SBIRT in Adolescent Population

SBIRT, as an intervention, also leads to changes in substance use behaviors among patients who were addicted to drugs and went through the SBIRT tool intervention plan. The large-cohort SAMHSA study in 2017, showed the effectiveness of the SBIRT intervention. The study was quasi-experimental and sponsored by the Substance Abuse and Mental Health Services Administration (SAMHSA, 2017). The study sample was about one million individuals, and the study participants were screened for substance abuse disorders, and the SBIRT was used as an intervention plan for five years. The use of SBIRT intervention process was linked with a decrease in substance use among the individuals who were under the SBIRT intervention process (SAMHSA, 2017). SBIRT is a critical intervention tool that is validated to help assess the risk of substance abuse and developing intervention to encourage the reduction of substance use and ensure treatment who have improve both physical and psychological issues resulting from substance use and misuse (Schmidt, 2010). Therefore, SBIRT is a standardized and evaluated screening tool that is effective, patient-centered, evidence-based and an ongoing transition to treatment and care of patients addicted to substance use.

Steps to Implement the Quality Improvement Initiative

            The first step of implementation of SBIRT intervention plan for patients engaged in substance abuse is to train the health care providers and physicians who will participate in the program. The health care providers who will be involved in the study will be reimbursed for every screening (SAMHSA, 2017). The SBIRT will include four steps during the screening process.

  1. The first step: The first step will involve the screening process where the health care practitioners will develop an evidence questionnaire to identify patient admitted for substance abuse ranging from addiction, problematic and riskiness of substance abuse. The screening process is critical in raising awareness to help people reduce substance use and misuse. The risk of substance use can be divided into four patterns: low risk or abstinence involves those who no consumption of alcohol, tobacco or any other drug. Driving risk includes those adolescents who use drug and alcohol and drive or ride with adolescents high on drugs or alcohol. Moderate risk includes individuals who have started using drugs and alcohol. Finally, high risk involves individuals who use drugs and alcohol often.
  2. The second step: This step involves a brief intervention process where teenagers showing mild symptoms get about 15 to 45-minute intervention including motivational interviewing. This helps to develop a behavioral change among teenagers to help reduce substance use and abuse.
  • The third step: The step involves a brief treatment process where adolescents with a score of moderate to severe in substance use and abuse are provided with a treatment plan that requires counseling sessions.
  1. The fourth step: The step involves the referral to treatment where the patients are connected to a physician for behavioral health treatment, comparative health assessment, or a specialty treatment program for substance abuse disorder.

These steps are critical during the implementation of the SBIRT as an intervention plan based on the following essential components: identifying adolescent at risk of substance abuse and disorders, educate adolescents and create awareness on substance abuse by showing the dangers of drinking and using drugs to help adopt a more healthy behavior (Schmidt, 2010). The potential for preventing substance abuse in the United States should not be underestimated. SBIRT is a critical intervention tool that helps in the reduction of substance abuse and its implementation as evidenced-based practices is an economic strategy that can help reduce abuse among teenagers in the US.

Antibiotics induce perturbations of the microbiota or dysbiosis, which in turn alters the host immune responses against pathogens. In comparison with adults, antibiotic treatments in infants have disproportionate consequences because the infant microbiota represents an evolving system that is unstable and immature until 2–3 years of age. However, relatively less knowledge is available on how antibiotics affect the infant microbiota and immunity.

In this review article, we focus on how Antibiotics and Childhood immunity and antibiotic treatment regimens influence the infant innate and adaptive immunity to pathogens in humans and animal models, and make the host susceptible to infections in later life. There is a critical need to better understand the effect of antibiotics on infant immune function, which may have implications for developing effective prophylactics and therapeutics against diseases in infants and adults. The consequences of disrupting this intricate balance during the early stages of life can be profound, affecting the trajectory of a child’s immune system and leaving lasting implications for future health.

The Gut Microbiome and its Role in Immune Function

Hidden within our bodies lies a thriving community of microorganisms known as the gut microbiome, which holds the power to shape our health and well-being in remarkable ways. Beyond aiding in digestion emerging evidence suggests that the gut may have an equally important role in the development and function of our immune system via both intestinal and systemic mechanisms. The gut microbiota is associated with the small intestinal paracellular permeability and the development of the immune system in healthy children during the first two years of life.

The gut microbiome is a diverse and dynamic collection of bacteria, viruses, fungi, and other microscopic organisms that reside in our digestive tract. This vibrant community interacts with our body’s immune system in a intricate dance, playing a crucial role in the development, regulation, and balance of our immune responses. The gut microbiome also directly influences systemic immunity in animal models, and modulation of the gut microbiome can impact the innate immune system in children. The gut microbiota provides essential health benefits to its host, particularly by regulating immune homeostasis. However, disruption to this delicate equilibrium can have far-reaching consequences. Factors such as antibiotic use, dietary choices, stress, and environmental influences can perturb the composition and diversity of the gut microbiome, potentially leading to dysbiosis—a state of imbalance within the microbial community. This dysbiosis has been linked to a range of immune-related disorders, including allergies, autoimmune diseases, and inflammation.

Fortunately, understanding the intricate relationship between the gut microbiome and immune function provides opportunities for interventions and preventive strategies. Researchers are now exploring the use of probiotics, prebiotics, and targeted dietary modifications to promote a healthy gut microbiome and optimize immune responses.

How Antibiotics Affect the Gut Microbiome and Immune Function

Antibiotics have undoubtedly revolutionized modern medicine, saving countless lives by combating bacterial infections. However, while antibiotics target harmful bacteria, they can also inadvertently disrupt the delicate balance of the gut microbiome, giving rise to a host of unintended consequences. Although the effects of prenatal antibiotics on neonates remain unclear, the microbes that first colonize a child after birth are known to have a fundamental influence on the development of the microbiome. An infant’s mode of delivery is a critical determinant of the composition of their gut microbiota.

The Long-Term Effects of Antibiotics on Childhood Health Outcomes

Antibiotics are amongst the most commonly used drugs in children, including infants, in the Western world. While antibiotics have transformed previously lethal infections into relatively minor diseases, antibiotic treatments can have adverse effects on childhood health outcomes. Here are some of the long-term effects of antibiotics on childhood health outcomes:

  1. Increased risk of wheezing and asthma: A systematic review and meta-analysis found that antibiotic exposure was associated with an increased risk of developing wheezing and asthma in children. Antibiotics prescribed from 12 to 24 months were associated with a significantly increased risk of asthma in both sexes, according to a study published in Mayo Clinic Proceedings.
  2. Increased risk of immunological disorders: Antibiotic exposure has been associated with an increased risk of allergy, various infections, and inflammatory bowel disease. Cephalosporins were associated with the most risk for multiple diseases, and uniquely autism and food allergies. Early antibiotic exposure has been associated with an increased risk of childhood onset asthma, allergic rhinitis, atopic dermatitis, celiac disease, overweight, and obesity. A study found that children under age two who take antibiotics are at greater risk for childhood-onset asthma, respiratory allergies, eczema, celiac disease, obesity, and attention deficit hyperactivity disorder. Although rates have declined in recent years, antibiotic use in children remains high; clinicians prescribed 67 million to US children in 2013. A growing body of evidence reveals early-life antibiotic use can have long-term consequences. In one study, the impact of antibiotic exposure in the first two years of life was studied in five-year-old children in Japan. Antibiotics were a risk factor for asthma, atopic dermatitis, and rhinitis in these children. These findings suggest that antibiotics can have adverse long-term effects on childhood health outcomes. It is important to use antibiotics judiciously and only when necessary to minimize the risk of adverse effects on childhood health. Healthcare providers should consider the potential impact of antibiotics and other medications on childhood health outcomes when prescribing them to children. Parents can also promote a healthy childhood by providing a balanced and varied diet, avoiding unnecessary antibiotics, and encouraging physical activity.

Adverse Event or Near Miss Analysis

Introduction

Adverse events are defined as injuries resulting in unnecessary and prolonged hospitalization, disability or even death as a result of healthcare mismanagement. On the other hand, a near miss is a serious error that could potentially result in an adverse event, but fails to do so due to an intervention or by chance (Rafter et al., 2015). These events have a significant impact on patients and result in increased healthcare costs attributed to longer stays in the hospital. Preventable medical errors are not limited to lower quality healthcare facilities; excellent facilities also experience undesirable events. What differentiates high quality facilities from low quality facilities is the way they respond to adverse events to significantly reduce the risk of the event reoccurring. Research shows that approximately 10% of patients will experience preventable harm during their stay in a medical facility. Over the years, health systems and governments have invested heavily in quality improvement systems to reduce harm to patients (Harrison et al., 2019). Additionally, incident reporting and analysis of adverse and near miss events plays a critical role in reducing and preventing reoccurrence.

Adverse Event or Near Miss Analysis

As a nurse who offers care to elderly patients over 70 years old, there are several adverse events that affect patient care and result in harm. Falls are a devastating and common complication in healthcare facilities particularly for older patients. Statistics shows that between 700,000 to 1 million hospitalized patients fall every year. A large percentage of patients who fall are in long-term care facilities (U.S Department of Health and Human Services, 2019). Furthermore, one-third of falls result in serious injuries including fractures, head traumas or even death. In line with this, the analysis will integrate research and data on falls, provide a real-life example in practice and recommend effective strategies to mitigate and prevent reoccurrence of falls in the healthcare facility.

The Adverse Event

The facility recently received a supply of modern mobility devices in form of walkers and canes. Among the elderly patients to benefit from the mobility devices was a 70-year old grandmother of four. All the beneficiaries of the devices received comprehensive training on how to use the walking aids. Additionally, the devices were fitted by professionals and every patient was assigned either a walker or a cane depending on the results of the professional assessment completed. The 70-year old grandmother was allocated a walker which was fitted to her unique needs taking into consideration the fact that she recently had a major hip surgery and required assistance to move around. However, during a shift change, there was poor communication between nurses and as a result, the patient was given a cane instead. The patient within a few minutes of using the cane, fell and broke her hip and pelvis. She was immediately rushed to the emergency room.

Analysis of Missed Steps Related to the Adverse Event

Research shows that over 47,000 emergency room visits for falls are related to the use of canes and walkers in individuals over 65 years old. Generally, older people using walking aids are at a higher risk of falling than those who do not use walking aids. This is attributed to spatiotemporal gait pattern, age and psychotropic drug intake. Therefore, extensive training and the use of appropriate walking aids greatly reduces occurrence of falls (de Mettelinge& Dirk, 2015). In addition to the high-risk factors associated with elderly falls, nurse errors played a critical role in the 70-year old grandmother’s fall-related injuries.

Clinical handovers occur in clinical settings between shifts. During these processes, professional responsibility over a patient is transferred from one healthcare professional to another, either on a temporary basis or permanently (Manias et al., 2015). The transfer of essential information is an integral part of communication in the healthcare setting. Ineffective handoffs such as the one experienced in the facility result in breaches in patient safety, which ultimately results in serious harm. There was a communication breakdown during the clinical handoff of the patient resulting in them receiving a cane rather than a walker. Since the mobility devices were new in the facility, there was little to no information about their use. Apart from the training given to patients by professionals, nurses were never trained on the appropriate use of these devices. Ineffective patient handoff and lack of staff training resulted in the patient’s fall and required emergency care.

While mobility devices have been known to reduce falls, other studies have questioned their efficacy. These studies claim that individuals who use mobility devices like canes or walkers may already have a predisposition to falls. To analyze the effect of adverse events on patients, their families, the healthcare facility and the community in general, several assumptions are made; stakeholders have a critical role to play in ensuring quality care is provided. The organization’s culture affects quality of care offered to patients and effective communication between care givers reduces the occurrence of adverse events.

The first victims of adverse events are patients and their families. When medical errors occur, patients risk experiencing life-changing events including; disability, extended hospital stays, physical harm or even death. Additionally, patients and their families experience psychological harm brought about by grief and avoidable patient suffering. Disability and death also result in economic stress on family members and caregivers (Holden & Card, 2019).

Healthcare professionals experience emotional distress following adverse events. Referred to as the second victims, healthcare professionals experience feelings of guilt, shame, fear, anger, disappointment and doubt when the patients they are caring for experience adverse events. Studies show that these feelings are long lasting and affect their future interactions with patients. Additionally, physicians report loss of self-confidence, increased anxiety about future errors and reduced job satisfaction (Ullstrom et al. 2014)

Evaluation of Quality Improvement Technologies

Communication patterns during shift transfers are ambiguous, problematic and non-standardized, thus contributing to increased patient harm. Attention should be focused on improving communication during patient handover. The use of standardized communication aids like the situation, background, assessment, recommendation (SBAR) tool improve communication between health care professionals (Gobel et al., 2014). When caregivers communicate with SBAR forms, two cue types of communication should be evaluated. They include; patient-situation cues that convey patient’s situation and background cues that convey specific data about the patient’s situation. Successful use of SBAR involves effective use of both cues without any data omissions (Gobel et al. 2014). Other communication devices like iSoBar checklist and TeamStepps team training tool significantly improve patient handover procedures and effectiveness. iSoBar handover forms are comprehensive and they reduce the number of clinical handover forms and prevent duplication of paperwork. They are easy to integrate into existing processes in the facility thus are well suited to local conditions.Likewise, the use of digital patient files and checklists in daily practice significantly reduce handover errors and incidences of miss-communication (Gobel et al., 2014).

Healthcare facilities are increasingly using health information technology (HIT) to identify and prevent the occurrence of adverse events. Health information technology facilitates communication between healthcare providers, improves tracking and reporting of events and promotes quality of care by optimizing access to care and adhering to guidelines. HIT improves patient safety through the use of clinical flags, reminders and medication alerts (CommitteeOpinion, 2015).HITs can be evaluated by using a synthesis framework that analyses the rate of falls before and after its implementation. Success will be determined by the rate of reduced falls in the facility. Technology-based interventionsdetect falls as soon as they occur and alert clinicians. Technology also allows older adults to self-assess for assistive equipment provision (Hamm, Money, Atwal & Paraskevopoulos, 2016). Health information technology (HIT) improves patient care through the use of effective frameworks and metrics.

Relevant Metrics of the Adverse Event

The data on fall-related injuries will be obtained from electronic medical records used by clinicians to monitor patient’s treatments. Additionally, data will be retrieved from the patients’ electronic health records. Organizational departments including the administrative and finance department will provide supplemental data. Numerical data will be obtained from patient surveys that capture self-reported information from the elderly patients about their healthcare experiences. Additionally, patient numerical data will be obtained from the facility’s Minimum Data Set (MDS) and the Outcome and Assessment Information Set (OASIS). Three KPIs will be analyzed including; outcome, process and structural metrics. To maintain patient safety, the healthcare facility must use meaningful metrics. Effective frameworks are based on two main metrics: one that identifies measures and eliminates errors and the other that identifies measures and eliminates injuries (“Measurement of Patient Safety”, 2019). The data obtained from these metrics can be used to inform improvement efforts, enhance transparency and maintain organizational accreditation.

To prevent future falls, the facility should invest in safety measurement strategies. Among the most effective safety measurement strategies is the retrospective chart review which contains detailed clinical information. Voluntary reporting error systems are tools used for internal quality improvement and highlighting adverse events. Automated surveillance is a helpful tool used to screen patients who are at a higher risk of experiencing adverse eventssuch as falls.

AHRQ patient safety indicators are useful for tracking adverse events over a long period of time and across large groups of people. Lastly, the facility should rely on patient reports to capture communication errors between healthcare providers and other errors that are not easily recognized (“Measurement of Patient Safety”, 2019). While the aforementioned patient safety improvement frameworks will assist the facility reduce fall rates and improve patient handoff practices, there needs to be a quality improvement initiative to prevent future occurrence of the adverse event.

Quality Improvement Initiative to Prevent Future Adverse Event

Quality improvement (QI) is a formal approach designed to analyze practice performance and the efforts directed towards improving it. QI models present a formal framework for establishing quality processes in healthcare facilities. Among the common QI models is the model for improvement referred to as the plan-do-study-act (PDSA) cycle. This model combines the total quality management model and the rapid cycle improvement model (AHRQ, 2015). The “plan” stage requires the healthcare facility to write a concise statement of what it plans to test and what steps it will follow to execute the plan. The “do” stage requires the facility to execute the plan it established during the plan stage. After implementation, the facility should study the results to determine if the interventions implemented are successful. Lastly, the “act” stage requires the facility to analyze the overall outcomes of interventions to determine future implications (AHRQ, 2015).

Quality improvement initiatives designed to affect favorable change can use the PDSA model. The model is the most favorable model as it establishes a causal relationship between changes, behaviors and outcomes. In line with the proposed steps-, the facility should start with determining the scope of fall rates, what changes should be made, who should be involved, what KPIs should be measured and where the QI strategy will be targeted. To reduce fall rates in the facility, a multi-component quality improvement program that involves clinician education and prevention of falls should be implemented. There are several reasons that result in elderly falls. As such, educating nurses, clinicians and physiotherapists on adherence to protocols and general safety practices including proper use of walking aids will help prevent future falls. To further improve process outcomes, bi-annual training activities should be conducted especially before new devices like walking aids are introduced. Additionally, improving communication between nurses during patient handover and conducting comprehensive assessments will prevent future occurrence of preventable medical errors. The QI program designed should prompt primary care givers to implement interventions like referrals to other community resources and screening programs for patients with a high fall risk (Ganz et al. 2016). The screening programs should analyze predisposing factors like the use of medication such as anxiety and antipsychotic drugs and patients’ age. Likewise, the facility should incorporate the CDC STEADI (Stopping Elderly Accidents, Deaths and Injuries) initiative consisting of three core elements: Screen, assess, intervene. STEADI’s main goal is to reduce the risk of elderly falls by offering tailored interventions

Measuring performance is essential to improving quality of healthcare. Effective QIs make use of available patient data. The PDSA model, when used in conjunction with health information technologies, will reduce the probability of future falls and improve communication between nurses, physicians and patients in the facility.

 

Conclusion

Man is to error. Healthcare providers are also human beings and therefore, at times, they make mistakeswhen administering care to their patients. Adverse events are very common in healthcare settings. As explained in the real-life scenario provided, elderly falls often result in life-threatening injuries. A majority of these falls are caused by inappropriate use of mobility devices. Poor communication practices between nurses during patient handovers also result in adverse events. However, by applying health information technologies and QI models, these errors can be controlled and prevented.
