This is often the most difficult part of any translational or evidence-based project. Your plan should include well-developed paragraphs for each of the following:

1. Your evidence-based intervention and evaluation of how you will create awareness and interest in your project in your unit/organization (Phase 1 of the model).

2. Steps you will take in your organization to build knowledge and commitment to your intervention (Phase 2).

3. Plan to promote action and adoption of your intervention (Phase 3).

4. Ways in which you will make your intervention/practice change sustainable after your project has completed (Phase 4).

You must cite any authors from evidence you used to support your plan. However, you do not need other references unless you use literature in addition to Cullen et al. (2022).
