Social determinants of health are nonmedical factors that influence a patientâ€™s health outcomes. Social determinants of health can be grouped into five categories which include economic stability, education access and quality, healthcare access and quality, neighborhood/environment, and social/community context (Office of Disease Prevention and Health Promotion, 2019).

Healthcare Access and Quality

In reviewing this patients SOAP note, the most concerning social determinant of health category that I feel is of importance is healthcare access and quality. Patient reports being seen in ED for pounding headache and having a very elevated BP, but not being on a current treatment plan for hypertension nor does the patient have a primary care provider. Patient is overdue for an eye exam and cervical cancer screening. Patient was also recommended different cancer screenings due to extensive familial history of different cancers. I feel this area is of priority because it focuses on patients ability and access to quality healthcare resources. There are many reasons people donâ€™t have access to quality healthcare which include provider availability, transportation issues, and health insurance issues (Office of Disease Prevention and Health Promotion, 2019).

Economic Stability

Another important category would be economic stability. With all of her new diagnosis patient may be subject to expenses related to new prescriptions, referrals to different specialists such as cardiology, further diagnostic testing, diet change, and smoking cessation. Patient also reports that she is recently divorced which can alter her financial situation. It is unknow whether ex husband is still financially supporting patient, but loss of that second income can impact a lot. She works in retail which doesnâ€™t pay much in this economy. She is also the parent of two children which can impact finances as well.

Social and Community Context

Lastly, I feel that there is a deficit in social support. Patient is recently divorced as stated earlier, so she no longer has the support of a spouse. Patient is suffering from many health issues herself but is trying to make a change to better herself for her children. I am concerned regarding her mental and social emotional state. Patient also has family members which she reports having significant medical illnesses. It is unknown if she is the caregiver to her family members (i.e. mother and father) as well.

Social determinants of health have a major impact on peopleâ€™s health and well-being, and affect their quality of life. Social determinants of health affect a wide range of health, functioning, and quality-of-life outcomes and risks based on the conditions where people are born, live, learn, work, play, worship, and age (Office of Disease Prevention and Health Promotion, 2019).
