Use your lecture materials to determine three priority Social Determinants of Health (SDH) to assess for in the patient represented in the SOAP note. In paragraph form construct a discussion that identifies those priority SDHs using data from the objective and subjective data sets where appropriate to support your discussion.

The patient was prescribed a medication that does not work with alcohol consumption. It raises blood pressure and needs to be discontinued. As the patient has high blood pressure and drinks 1-2 times a week, Fioricet was not a good medication choice for the patient. Fioricet is a combination of butalbital, acetaminophen, and caffeine. Acetaminophen and alcohol increase the chances of liver damage. Alcohol consumption with Fioricet increases the feeling of being intoxicated. This can lead to extreme dizziness and fainting. This medication can lead to addiction (Deveney, 2023). The patient is experiencing hypertension. Fioricet has caffeine as part of its active ingredients, and it causes blood vessels to constrict and increases blood flow. This increases the blood pressure (Banyan Treatment Centers, 2023). As the patient has high blood pressure and a family history of hypertension, Fioricet is also not a good choice for the patient.

Three social determinants of health (SDH) would be health care system-quality of care-prescription, health care system-quality of care-education, and economic stability-income and support (Office of Disease Prevention and Health Promotion, n.d.). Health care system-quality of care relates to the prescription of Fioricet. The prescriber did not look at the patientâ€™s history of social history and prescribe a medication that is conducive to her. This resulted in poor quality of care. The second SDH was health care system-quality of care related to being properly trained on the blood pressure cuff readings. The patient lacked the training by a medical professional on what the cuff readings mean and how to respond and react to those readings. The third SDH is economic stability-income and support. The patient recently went through a divorce and is caring for 2 children. She works in retail, which does not provide a stable suitable income for supporting a family of 3. She smokes and drinks, which is a coping mechanism. She has mild depression. She needs economic stability and emotional support.

Include in your discussion some strategies you might utilize to address challenges with or support strengths identified in those three priority SDH.

The patient needs guidance medically, economically, and mentally. Her medication was changed which was a better quality of care. The plan of her keeping a blood pressure log is conducive to training the patient on her cuff and helps her to understand the numbers of her readings and when to seek help. An educator should be scheduled with the patient to help address any issues she has with her health. A social worker needs to be assigned to the patient to help address financial and emotional concerns to help her get the help she needs. It appears that she may be overwhelmed with her new role of being the primary caregiver without support. Single parenting is mostly single mothers. Single-mother households are 85% of single-parent households. The rates of these homes have significantly risen from 1990 to 2019 by 20%. Single-father households comprise less than 2%, with an increase of 3.4% from 1990 to 2019 (HÃ¤rkÃ¶nen et al., 2023). There needs to be better and easier ways for single-parent households to find resources and support.
