1. Insomnia

Plan (P):The patient presents with insomnia which can be caused by many life stressors. The goal for plan of care is to provide relief for insomnia with creating a healthy sleep environment. 

Diagnostics: It is noted, “insomnia and depression have been known to be risk factors of several diseases, including coronary heart disease” (Jung et al., 2023, p. 1). Provider should assess how often sleep difficulty occurs. A sleep study can be ordered to diagnose sleep disorders. CBC, CMP, and thyroid panel to check for thyroid problems that can affect sleep.

Therapeutic:Take 1mg of Melatonin two hours before bed.

Educational: Remove electronic devices, such as TVs, computers, and smart phones, from the bedroom. Avoid large meals, caffeine, and alcohol before bedtime. Get some exercise and maintain healthy weight. Melatonin can cause side effects including headache, sleepiness, dizziness, and nausea. Follow up in one month to review insomnia. 

Consultation/Collaboration: Referral to a sleep specialist. 

2. Obesity

Plan (P):

The interventions presented are to conquer obesity through healthy lifestyle changes. It is noted that “there is a pressing need to address obesity at all levels of the healthcare system as the prevalence of obesity increases globally” (Yunus et al., 2023, p. 2).

Diagnostics: CBC, CMP, and thyroid panel to check for thyroid problems. Check A1c for diabetes. Check lipids and cholesterol. 

Therapeutic: No changes in medications. Weight loss regimen and food diary. 

Educational: Limit fast food, sweets, and processed snack foods. Choose low-fat options, such as low-fat milk instead of whole milk. Eat more servings of fruits and vegetables every day. Exercise at least three times a week.

Consultation/Collaboration: Referral to a nutritionist for weight loss.
