Debbie O’Connor is a 55-year-old female who has been previously diagnosed with Group B COPD, but whose symptoms indicate worsening of her COPD to Group E. Her 02 Sat is 92% on room air, and her RR is 20. Her CAT score is 17. She is currently a smoker and smokes 3 – 5 cigarettes a day. She has tried nicotine replacement in the past but was unsuccessful. She experienced a bronchospasm three days ago and has had increased fatigue and difficulty breathing since then. She does not have any inhaler for symptom exacerbation. She has expressed a desire to quit smoking, which is vitally important in order to improve her condition.

I prescribed Ms. O’Connor • a tiotropium dry powder inhaler, a long-acting muscarinic antagonist for her COPD • a short-acting albuterol / ipratropium combination treatment for her COPD exacerbation symptoms • varenicline as a smoking cessation aid Ms. O’Connor should receive an annual influenza vaccine during flu season. She should also receive a pneumococcal vaccination. Encouraged Ms. O’Connor to quit smoking to alleviate her bronchospasms. Provided her with information on community smoking cessation programs to help support her in this endeavor. Reinforced the need for a healthy diet and light exercise to alleviate her COPD symptoms. Ms. O’Connor needs to schedule a follow-up appointment in the next four weeks in order to evaluate her new COPD therapy and smoking cessation aid. I recommended that she begin checking her spirometry reading daily and keeping a log of her pulmonary function readings to bring to her next appointment.
