Underlying Pathophysiological Mechanisms of Anemia
Iron deficiency anemia is a state in which iron stores are depleted and inadequate to meet the needs of hemoglobin synthesis, leading to decreased tissue oxygenation (hypoxia) and reduced oxygen-carrying capacity. In women of reproductive age, chronic blood loss due to gynecologic disorders like endometriosis is a frequent occurrence since the continual bleeding gradually leads to a decrease in iron storage and suppression of erythropoiesis (Auerbach, 2023; Pasricha et al., 2021). As hemoglobin levels drop, so does the amount of oxygen delivered to tissues, triggering compensatory cardiovascular and respiratory responses that can cause fatigue, weakness, dizziness, and shortness of breath. Tissue hypoxia resulting from poor hemoglobin formation and iron deficiency causes Janessa to feel weak, faint, and breathless, with pale mucous membranes (Iolascon et al., 2024). The paleness of Janessa’s mucous membranes is another physical symptom of low hemoglobin levels and decreased oxygenated blood in the superficial tissues, characteristic of iron deficiency anemia (Iolascon et al., 2024). These mechanisms are very similar to what she has presented.
Analysis of Janessa’s Clinical Manifestations
The clinical findings are highly suggestive of iron-deficiency anemia, not only in light of the underlying pathophysiology but also given Janessa’s medical history. She has had a history of weakness, dizziness, and dyspnea for the past month, along with pale mucous membranes and a history of endometriosis, indicating chronic blood loss and iron depletion, rather than an acute disease (Auerbach, 2023). The lack of cough, fever, or lung sounds also reduces the likelihood of a primary respiratory problem. It allows the focus to be directed toward a hematologic etiology of her symptoms (Pasricha et al., 2021). Other possible differential diagnoses include vitamin B12 deficiency or chronic disease anemia, but her presentation is suggestive of iron deficiency anemia (Iolascon et al., 2024). The overall clinical evidence favors the diagnosis of anemia.
Diagnostic Evaluation for Iron Deficiency Anemia
A complete blood count, iron studies, peripheral blood smear, and reticulocyte count should be performed to confirm and quantify iron deficiency anemia as part of the initial workup. A full blood count would show reduced hemoglobin, hematocrit, and MCV, and iron studies would show low ferritin, low serum iron, high TIF, and low transferrin saturation (Iolascon et al., 2024; Pasricha et al., 2021). The peripheral smear would likely reveal microcytic hypochromic red cells. At the same time, the low reticulocyte count would suggest that the bone marrow was not producing enough cells due to iron deficiency. Imaging is not standard for diagnosis, but evaluation of the female genitals might be warranted to determine if chronic bleeding from endometriosis is the cause of iron loss (Auerbach, 2023). The diagnostic results would help determine the presence and cause of Janessa’s anemia.
