Overview
As a practitioner it is important that clinical recommendation be based on the best available evidence. Practitioners not only need to be able to assess the quality of the literature but summarize and grade the available literature in order to make sound clinical decision for or against a course of treatment. The goal of this assignment is to a use a standardized assessment form to critically appraise a topic (based upon an approved PICO-style clinical question) and develop clinical recommendations for or against a treatment based upon the best available evidence.
Instructions
Using your approved PICO-style clinical question and the 3 previously submitted study appraisal (updated to reflect the recommended changes by the instructor) complete the Critically Appraised Topic (CAT) assessment template. The template consists of three parts (Part I, II and III). For part I, the student will provide 1-2-word subject title, PICO question, search strategy (database and search terms used), number of retrieved articles, and APA style reference list for the 3 articles appraised for quality. In part II, students will address questions related to the quality of the evidence reviewed (i.e., the 3 articles appraised for quality) as well as questions related to the evidence for the recommendations. In part II student will provide recommendations for or against the intervention a long with a justification for the specific recommendation.
Recommendations should be framed as the following:
· Strong recommendation against
· Conditional recommendation against
· Recommendation for research and possibly conditional recommendation for use restricted to trials.

For example, “Based on the patient values and current health status, there is a strong recommendation for the use of NSAIDs in the treatment of joint pain.”
All justification should be consistent with the recommendation. Justification should be framed as the following italicized portion of the statements below:
· If strong recommendation against – Undesirable consequences clearly outweigh desirable consequences
· If conditional recommendation against – Undesirable consequences probably outweigh desirable consequences
· If recommendation for research and possibly conditional recommendation for use restricted to trials – Balance between desirable and undesirable consequences is closely balanced or uncertain
· If conditional recommendation for – Desirable consequences probably outweigh undesirable consequences
· If strong recommendation for – Desirable consequences clearly outweigh undesirable consequences
For example, “Given the patient’s age and health status the desirable consequences (such as effective pain mediation and reduction of inflammation) clearly outweigh undesirable consequences (i.e., indigestion, stomach ulcers, allergic reaction and in rare cases problems with your liver, kidneys or heart and circulation).
Finally for part III, the student must provide a 2-page literature review in APA style using the three articles appraised for the assignment. The 2-page literature review should immediately be followed with an APA style reference list. The literature review should include the following sections:
I. Background/Clinical Scenario – In this section provide description of the background of the problem or clinical scenario leading to the clinical question. End this section, with a statement of the approved clinical question.
II. Summary of Key Findings – In this section state all the key clinical findings from the appraised evidence.
III. Level of Evidence, Quality, and Grade – In the section, also provide a brief review of the quality and level of evidence provided by each article. End this section, with a statement indicating the grade recommendation for the retrieved evidence.
IV. Clinical Recommendation – In this section, indicated the clinical recommendation for or against as well as areas of future research.
V. References – APA style reference list required.
